this is my first concept and i know its not balanced, but im trying to make it balanced, also sorry for my bad english.
feel free to give me feedback and suggestions.
p.s.i know this doesnt make much sense with the lore.

Hello.
Chaos energy has value of a heroic ability;
5 second silence on dispel(which becomes 7 with talent) is too long for Hots, as well as 10 second dot, 10 second slow and 5 second invulnerability;
5 second aoe stun? That will end the game. It's ultraoverpowered even if it was a heroic and dealt no damage;
Cure already exists in the game but is tier 3 talent and called Cleanse, Scouting drone is tier 1 talent, not tier 2, Battle momentum is tier 3, not tier 5;
Only tier 7 talents should affect heroics, besides why would you take a talent at 4 which will do nothing until 10 level;
Again stun for 5 seconds is too long;
Medic is basically Brightwing's trait, only probably 100 times better and as a talent.
I'm also not sure if he doesn't have the ability to use mounts or Stealth is another trait.
As you said, it doesn't fit lore. Medivh has nothing to do with Inferno and definitely wasn't an assassin, so I'm not sure why you chose him for this particular concept. It's unclear what this character needs to accomplish in game. Also, assassin creating an earthquakes doesn't sound right, as well as becoming invulnerable after you charge at the enemy.
I suggest doing a research on a character before creating a concept, so abilities fit his lore and form a cohesive kit together. It would also be better if you compare abilities you want to make with similar existing ones, to make them more balanced.

Interesting idea, here's some suggestions:

Lower the stacks from the combat trait but also reduce the healing to a solid amount (All the heroes have solid controlable stats so no need to make percentage amounts)
Dispel's silence duration is too long for a point-and-click ability (Earthquake too).
Change his Bless a little, it's too similar to Uther's Q (maybe add an hots or a damage reduction for a while)
The Rush's invunerability is too strong too, give it a damage reduction and lower that duration.

For the Heroics I'd suggest to change his Assassin Form; abuse of his staff (Atiesh), give him some more "crowish" set or appearance.
