1) 100% of the time you take Reanimation, it gives you that regen, and it brings down your death timer based on your stacks.

4) Hardened Bones works well to keep that damage down, plus you can use it to body block with your ult, and take less damage

7) Toss up, I like Ghastly Reach, for working better to clear waves, and to hit more target with Consume Vitality later on and get health back, but Ossein Renewal can work, I'm just not a fan

10) Entomb.

13) Drain Momentum, another toss up, I personally like not being slowed by this, since your already so ungodly slow, Burning Rage is viable, as with most tanks, and maybe Spell Shield based on the enemy team.

16) Consume Vitality, it's just better than all the rest, if you got the increased range on your "Q" then you should easily heal for 8-10% each hit.

20) Hardened Shield, just a good tank talent, especially to activate when you wall the opposing team. You attack to slow for Spectral Leech
